Admission Forms
There is a bit of paperwork, but we are trying to make it as easy as possible through our online download, and hopefully save some trees while we are at it. Each parent/guardian is asked to read thoroughly and sign our:

This will inform you about the school, our philosophies, general policies, tuition schedule/fee, and more. You will also need to sign off on this!

The yearly update form must be updated annually. To make this easier for you, we will try to encourage these are filled out in September of each year; however, if they are not turned in we will ask that you update it annually based off of their enrollment date.
Medication Administration-
Medication Administration Permission
When signing our “General Health Appraisal”, you have the option to sign for the administration of Infants/Children’s Tylenol or Motrin. By signing this, only our trained staff may administer Tylenol or Motrin upon your request and with your signature on the given day. Please keep in mind, that this would be Tylenol or Motrin provided from home, labeled with the child’s name and in the original bottle.
With that said, if you have signed this form, you will not need to complete the entire form- only the top portion of the form attached (we will complete the bottom with the “General Health Appraisal” form you signed).
If you would like any other medication administered to your child (including prescription and over the counter creams and medications)- we must have a doctor’s note. The doctor’s office can fax it to us or you can drop it off- and we will then complete the bottom portion of the form attached with that information. You will still need to complete the top portion.
Medications that we will need a note for include (but are not limited to):
-Vaseline
-Hydrocortisone cream
-Any Antibiotics
-Cough medicine
-Teething tablets (even homeopathic)
-Neosporin
